Abstract
Objective: To evaluate the effects of tadalafil (20 mg) on the pharmacokinetics of the drug in healthy men with erectile dysfunction. Methods: Healthy subjects received tadalafil (20 mg) once a day for a period of 12 weeks. Following the 12 weeks of tadalafil treatment, the mean plasma concentrations of tadalafil in the normal men were not significantly different from those in the men with erectile dysfunction. The plasma concentration-time profile of tadalafil was consistent with the pharmacokinetics of the drug. Results: In the normal man, the mean plasma concentrations of tadalafil in men with erectile dysfunction were significantly higher than those in men with normal sex hormone-binding globulin (SHBG) concentration.
Conclusions: Tadalafil is a potent inhibitor of the bioavailability of tadalafil in men with erectile dysfunction. Tadalafil should be used with caution in men with erectile dysfunction.

Tadalafil, a phosphodiesterase type 5 (PDE5) inhibitor, was originally approved for the treatment of erectile dysfunction (ED) in 2003. The original indications for tadalafil include the treatment of ED by improving blood flow to the penis, and its vasodilatory effects, which are thought to be mediated by the PDE5 enzyme.
Tadalafil is not approved for the treatment of ED in men, even though it is used for the treatment of ED in women. The FDA has approved tadalafil for the treatment of ED in men. Tadalafil is a PDE5 inhibitor and is not associated with any side effects.
Tadalafil has been studied for its effectiveness in the treatment of ED in men. However, tadalafil has been associated with increased rates of arterial hypertension, which can be improved by blood flow enhancers, such as sildenafil (Viagra®), tadalafil or vardenafil (Levitra®).
The use of tadalafil has been associated with increased hypotensive effects, such as headache, flushing, dyspepsia, and muscle aches, which are commonly caused by ED.
However, there are no data regarding the effects of tadalafil on men with erectile dysfunction. The current study aimed to evaluate the effects of tadalafil on the pharmacokinetics of the drug in healthy men with erectile dysfunction.
This was a 2-period, double-blind, placebo-controlled, randomized, multicenter trial that enrolled 50 healthy men with erectile dysfunction (ED) (mean age, 48.6 ± 10.1 years) and normal sex hormone-binding globulin (SHBG) concentration (mean ± SD, 18.5 ± 2.5 mL/L). After the first 2 weeks, all the men were treated with either tadalafil or placebo. After a period of 12 weeks, blood samples were taken for the determination of plasma concentrations of tadalafil.

If you experience any of the following side effects stop using the medicine and seek medical help immediately:
allergic reactions including rashes (frequency uncommon). chest pain – do not use nitrates but seek immediate medical assistance (frequency uncommon). priapism, a prolonged and possibly painful erection after taking CIALIS (frequency rare). If you have such an erection, which lasts continuously for more than 4 hours you should contact a doctor immediately. sudden loss of vision (frequency rare). Other side effects have been reported:

headache, back pain, muscle aches, pain in arms and legs, facial flushing, nasal congestion, and indigestion. Uncommon (seen in 1 to 10 in every 1,000 patients)
dizziness, stomach ache, feeling sick, being sick (vomiting), reflux, blurred vision, eye pain, difficulty in breathing, presence of blood in urine, prolonged erection, pounding heartbeat sensation, a fast heart rate, high blood pressure, low blood pressure, nose bleeds, ringing in the ears, swelling of the hands, feet or ankles and feeling tired. Rare (seen in 1 to 10 in every 10,000 patients)
fainting, seizures and passing memory loss, swelling of the eyelids, red eyes, sudden decrease or loss of hearing, hives (itchy red welts on the surface of the skin), penile bleeding, presence of blood in semen and increased sweating. Heart attack and stroke have also been reported rarely in men taking CIALIS. Most of these men had known heart problems before taking this medicine.
Partial, temporary, or permanent decrease or loss of vision in one or both eyes has been rarely reported.
This medicine is not approved for use in the UK.

I also think that the new drug in Seattle will be a great addition to the pharmacy, and I will keep an eye on it as I see other options.
